1. FULL DISCLOSURE

I, the undersigned, have truthfully and completely informed the stylist at Hair Cuts At The Palms of all previous treatments performed on my hair, including but not limited to:

Box color/Home dye

Henna or metallic salts

Perms or relaxers

Previous bleaching or lightening

Any known allergies to hair products, latex, or chemicals.

I understand that withholding information regarding my hair history can lead to unpredictable results, hair breakage, or severe damage.

2. ACKNOWLEDGMENT OF RISK

I understand that chemical services (color, bleach, perms, keratin) involve the use of professional chemicals that may cause:

Skin irritation or allergic reactions.

Changes in hair texture or integrity.

Unexpected color results due to underlying pigments or previous chemical history.

3. CLOTHING & PERSONAL PROPERTY

I understand that while the salon provides capes and towels, Hair Cuts At The Palms is not responsible for any damage to clothing or personal items (jewelry, bags, etc.) caused by color, water, or chemicals during the service. I am advised to remove expensive jewelry or clothing that may be at risk.

4. SATISFACTION & ADJUSTMENTS

I understand that hair services are subjective. If I am not satisfied with the results, I agree to notify the salon within 48 hours of my appointment. Adjustments will be made at the discretion of management. I understand that “change of mind” requests (e.g., asking for a different color than originally requested) are not covered as free adjustments.

5. RELEASE OF LIABILITY

I hereby release Hair Cuts At The Palms, its owners, and its employees from any and all liability for any hair damage, physical reactions, or personal injury that may occur as a result of the services provided, provided that such services were performed with professional care. I waive my right to bring any legal action or claim against the salon for results that are inherent risks of chemical hair processing.

6. PHOTO RELEASE (Optional)

[ ] I grant permission to Hair Cuts At The Palms to take and use photos of my hair for promotional/social media purposes.
